To make a convenient appointment, simply call us today. Our practice is dedicated to identifying the cause of your pain and creating a customized treatment program that will get you back to the things you enjoy as quickly as possible.

Before Your First Visit

  • Make a list of questions that you would like to discuss on your first visit.
  • List your signs and symptoms for your physical therapist.
  • Please call our office so we can verify your insurance benefits.
  • If you have had any x-rays, CT scan, or MRI for the condition we are treating, please contact our office so we can obtain the results prior to your appointment.
  • If we are treating you following a surgery, please contact our office so we can obtain your surgery report prior to your first visit.
  • Arrive at the appointed time (no need to arrive early).
  • Make sure to wear comfortable clothing that you could exercise in. If we are treating your knee, hip or ankle, please wear or bring a pair of shorts.
  • Bring with you a copy of your insurance card, referral from your doctor if you have one, a photo ID and your appointment book so we can schedule needed follow-up visits.
  • In case of an automobile accident or worker’s compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers and claim number.
  • If you have any questions or concerns about your first visit, please call us! We are happy to help you in any way!

Forms to Complete Before Your First Visit

  • Complete the Medical Questionnaire and be sure to include key past medical history, previous treatments, current medications, and other important information.
  • Complete the outcome form that our office will email to you.
  • Complete the STarT Back form if we are seeing you for lower back pain.
  • Complete the Generic STarT Back form if we are seeing you for any condition other than lower back pain.

What to Expect on Your First Visit

  • You will check in with our friendly front office staff. They will help you navigate the insurance process and treatment program so you can focus on your recovery.
  • You will meet with your physical therapist for the initial evaluation.
  • During the evaluation, your physical therapist will ask questions about your medical history, current pain, or injury. Open communication helps us understand your problem.
  • Your physical therapist will then complete a thorough evaluation. This may include assessing your movement, strength, flexibility, joint mobility, coordination, nerve function, posture and much more.  Our purpose is to help you, so please ensure you communicate to your physical therapist if anything is hurting or bothering you during the examination.
  • Your physical therapist will the explain the causes of your symptoms, recommend and prepare a course of treatments to ensure the quickest recovery and the best results. Your physical therapist will discuss these recommendations with you.
  • At that point, your therapist will answer all of your questions and prescribe an individualized home program consisting of exercises and other techniques that will get you on your road to recovery.
  • There may be some treatments performed the first day to get you started. However, during the next visit you will get into the full swing of your treatment plan.
  • You will schedule your series of visits with the front desk. The amount of therapy needed will be discussed with you, depending on the recommendations of your physical therapist. We offer many convenient appointment times, however prime times can become busy, so it is best to book ahead as much as possible.
  • Your physical therapist will write up their findings and treatment plan to submit to your referring physician or other medical professional who you choose.
