Utah Physical Therapy Specialists is a unique, high quality, niche practice that focuses on your recovery. It is primarily a cash practice but does accept some insurances. Dr. Rodriguez provides one-on-one, personalized physical therapy tailored specifically to your needs. You will always be treated directly by him — a licensed, board-certified physical therapist — never by an aide, assistant, or technician. This ensures that every session is focused, high-quality, and effective.

Insurance & Payment Information

Dr. Rodriguez accepts the following insurance plans:

• Auto
• Blue Cross Blue Shield
• Direct Care Administrators
• DMBA
• Educators Mutual Insurance (EMI)
• Medicare
• Motivhealth
• Workers Comp

Dr. Rodriguez is an out-of-network provider for all other insurance plans.

If he does not accept your insurance, you still have options:

• You can pay out-of-pocket for your sessions.
• If your insurance plan offers out-of-network benefits, you may be able to get reimbursed directly by your insurance company.

In these cases, Dr. Rodriguez will provide you with a detailed superbill, which you can submit to your insurance for potential reimbursement.

Initial Exam:                $150.00                                            

30 Minute Session:   $75.00                                              

45 Minute Session:   $113.00                                            

60 Minute Session:   $150.00                                          

Packages (1 Hour Sessions)

         4 Visits:  $560.00

         6 Visits:  $810.00

         8 Visits:  $1,040.00

         10 Visits:  $1,250.00
